Recording the charges for printing, the number of copies printed and the number distributed to each diocese for Forms of Prayer including: "An Accompt how many Forms of Prayer for the general fast were deliver'd to His Grace the Arch-Bishop of Canterbury, and to the rest of the Lords of Bishops . . . 10 April 1697" (Wing C4162); proclamation for the fast (Wing W2455); "Forms of Prayer to be us'd during the time of His Majesty's absence . . . deliver'd . . . 22 April 1697" (Wing C4141A); "Forms of Prayer to be us'd . . . until the day of the public fast . . . deliver'd . . . 11 December 1703"; "Form of Prayer for the general fast . . . deliver'd . . . 19 December 1703."
